University of Arizona Cooperative Extension Content-Use Permission Request
Extension personnel have developed a blanket template to use when requesting to use content from other authors or publishers. These can be images, graphics, or written content to be included in UA Cooperative Extension publications. In many cases, written permission is not required other than a confirmation e-mail from the original author or owner of content. However, in those rare situations, the attached template was designed for your use.
Download and fill out completely and send to the owner(s) or author(s) of the content being requested. This is to protect you and Cooperative Extension from allegations of unauthorized use of content. You do not need to submit the returned form - keep for your records electronically.
