L. Collecting and submitting the background check packet

After the potential 4-H volunteer or Master Gardener volunteer receives their Arizona Department of Public Safety background check card, they come to the local office (or regularly scheduled meeting) with

  1. DPS Class 1 or IVP Fingerprint card
  2. Government issued photo id (driver license, passport, etc.) 

The volunteer

  1. Fills out the Fingerprint Clearance Card Applicant Authorization
  2. Signs the Authorization (allowing us to verify the status of the card)

Please note:  Human Resources requires pen-and-ink signatures on the document, or else AdobeSign verification of the signature for both the volunteer and the staff person.  

The staff person

  1. Verifies the identity of the volunteer
    1. ID matches person
    2. Name on DPS card and ID matches
  2. Signs the form that they have verified the identity
  3. Scans
    1. The completed and signed Fingerprint Clearance Card Applicant Authorization form
    2. DPS/IVP card front (and back if there are any restrictions listed)
    3. Photo ID front
  4. Saves file with name: LastName FirstInitial DPS Check.pdf

NOTE: For a 4-H volunteer who is new or is re-starting their DCC, the county office person sets up the transaction to initiate the DCC appointment and uploads the background check packet.

NOTE: Master Gardener volunteers will have already completed their DCC when they get their AZDPS card.  The packet should be submitted directly to gblumanhourst@arizona.edu  

The verification of validity of the background check is made in the State Cooperative Extension Volunteer Systems office and documentation is affixed to the transaction and then forwarded to HR.  If the background check card is not valid, the state office will notify the person who initiated the transaction and the sponsor for the volunteer.
